Step 1: Send in an inquiry

Step 2: We will reach out, create your client portal and send a few documents to complete. Please put aside at least 30 minutes to complete these forms and upload a few documents. 

Forms include:

Client Handbook, Intake interview questionnaire

Requested documents to upload within our HIPAA Compliant EHR: 

Drivers License, Insurance card, and any custodial paperwork.

Step 3: Verification of insurance (if applicable) and informing of any financial responsibilities...Please allow 3 days to verify you insurance coverage. 

Step 4: Scheduling of Intake session.
