Step 2: I will reach out via email, create your client portal and send you the Pre-Verification Document. This form is used to verify your insurance and financial responsibilities 

Step 3:Complete a Pre-Verification Document.

Step 4: Verification of insurance (if applicable) and informing of any financial responsibilities...Please allow 3 days to verify you insurance coverage. 

Step 5: Complete forms and Upload requested Documents within Client Portal

Forms include:

Client Handbook, Intake interview questionnaire

Requested documents to upload within our HIPAA Compliant EHR: 

Drivers License, Insurance card, and any custodial paperwork.

Step 6: Scheduling of Intake session.
